Which data must you send?

You must send us the following data, among other things:

• the name and address details of you as the borrower
• the name and address details of the lender (loan provider)
• your citizen service number
• the citizen service number or Foreign Tax Identification Number (TIN) of the lender
• the starting date of the loan
• the amount of the loan
• the term in months
• the interest rate
• the manner of repayment (on an annuity basis or by equal amounts)

Don't forget to place your signature on the data. If there are 2 borrowers, you must both place your signature.
