Form 'Opgaaf werkelijk rendement' – where can I find it?

Have you received a letter from us saying that you can report your actual return? Then you can fill in the form 'Opgaaf werkelijk rendement' (Submit Actual Return) for that year. You can do so online in Mijn Belastingdienst (the online portal of the Tax Administration), but you can also do so on paper. When filling in the form, keep our letter at hand, because it has information you need.

Form 'Opgaaf werkelijk rendement' 2020–2024

The online forms for the years 2020 to 2024 are available to you in Mijn Belastingdienst. You have to fill in the form there, you cannot download it.

Choose the year for which you want to report your actual return. Log in and click on 'Mijn werkelijk rendement box 3 opgeven' (Report my actual return for Box 3) to start the form.

Form Opgaaf werkelijk rendement 2017–2019

Do you want to report your actual return for 2017, 2018 or 2019? In that case, you can fill in the form right away with your DigiD. You don't need to log in to Mijn Belastingdienst.

Choose the year for which you want to fill in the form:

Submit actual return 2017 (only available in Dutch)
Submit actual return 2018 (only available in Dutch)
Submit actual return 2019 (only available in Dutch)

Do you prefer a paper form?

You can request a paper version of the 'Opgaaf werkelijke rendement' form by phone at 0800 – 023 01 07. When calling from abroad, use the Tax Information Line for non-Resident Tax Issues: +31 55 5 385 385. Keep in mind that the paper form does not show whether your actual return is lower than your notional return. That is only possible in the online form.
